Fill out the required information.

You will need:

1.  Your Application Number (from your ZAPBC confirmation email and/or confirmation web page download).
2. Your last name – the one you used on your application to our program.
3. The email address you used on your application; and,
4. The Serial Number for your charger (can be found on the UPC Code and Serial Number sticker photo you took in order to submit a claim).

NOTE:

Providing accurate information will expedite the process and ensure you receive your rebate faster! 

Your name and email address should match the name and email address you provided in your application.

Please take care to type in your application and serial number accurately.
